Understanding Your FHO+
FHO+ (Family Health Organization Plus) is the enhanced primary care model that Cityview Medical Centre has transitioned to as of July 1, 2026. An FHO+ is a group of family physicians working together — established by the Ontario Medical Association and the Ontario Ministry of Health — to give patients better access to quality primary health care.
At Cityview Medical Centre, Drs. Awais and Rawat are part of the Bolton-Brampton-Vaughan FHO+ — a seven-doctor group spanning three clinic locations in Vaughan, Brampton, and Bolton. During after-hours, urgent care is provided by whichever doctor from our FHO+ group is on call that day. That doctor has access to your medical history and can provide informed, connected care.
Previously, your care was delivered under a Comprehensive Care Model (CCM), where your doctor worked independently. Under FHO+, your doctor works as part of a formal group practice — giving you access to a broader network of physicians for urgent after-hours care, while still keeping your own family doctor for routine visits.
The key benefit: outside clinic hours, an on-call doctor within our FHO+ group — who has access to your medical history — will assist you, rather than an unfamiliar walk-in clinic physician.
As an enrolled patient, you benefit from:
  • A dedicated family physician who knows your full health history
  • Coordinated chronic disease management and preventive care
  • After-hours urgent care through our FHO+ on-call rotation (Mon–Thu 5–8 pm; Sat or Sun 10 am–1 pm)
  • Referrals to specialists managed and tracked by your doctor
All seven doctors in our FHO+ group practice across three locations — Vaughan, Brampton, and Bolton. By forming this partnership, we offer extended after-hours coverage seven days a week that no single clinic could sustain alone. Whichever clinic is on call, the doctor will have access to your medical records and can provide safe, informed care.
Your Care & Enrollment
Yes. If you are rostered with Dr. Awais, she remains your family physician. If you are rostered with Dr. Rawat, he remains your family physician. The FHO+ transition only changes the practice model and how after-hours coverage is organized — your relationship with your own registered doctor is unchanged.
No action is required from you. Existing patients are automatically transferred to the new FHO+ model. You do not need to sign any consent forms, complete enrollment paperwork, or update your OHIP information. Your care continues without interruption.
No. Existing prescriptions, specialist referrals, and ongoing care plans will continue exactly as before. The FHO+ transition is an administrative and billing model change — it does not affect any active treatments or care arrangements you have in place.
All three clinics use a shared electronic medical record system. Whichever FHO+ doctor you see — at Cityview in Vaughan, MayMc in Brampton, or BBV in Bolton — they will have access to your relevant medical history.
Your privacy is protected. All sharing of medical information across our group is governed by Ontario's Personal Health Information Protection Act (PHIPA) and is limited to physicians directly involved in your care.
Participation in the FHO+ is part of how care is delivered at Cityview Medical Centre going forward. Patients who choose not to remain enrolled are welcome to seek care with another family physician. Please contact our office if you have questions — we are happy to discuss your options.
Access & After-Hours Care
We strongly encourage you to contact Cityview Medical Centre between 10:00 am and 5:00 pm, Monday to Friday, for routine and same-day needs. For urgent care between 5:00 pm and 8:00 pm, Monday to Thursday, one of our on-call doctors will be available. An on-call doctor is also available Saturday or Sunday from 10:00 am to 1:00 pm.
Exception: Please only use an outside walk-in clinic on a Friday evening, or on a Saturday or Sunday when our FHO+ group is unable to provide cover or when our clinics are closed.
If your own doctor is away, another doctor at Cityview Medical Centre may be able to provide care during clinic hours. Please call us at 905-553-4499 and our team will do their best to accommodate you.

If our group cannot provide cover — such as on a Friday evening or when all clinics are closed — please visit your nearest urgent care clinic or emergency department.
